We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. Data protection
All information held by the Practice will be treated as highly confidential and will not be shared with any third party without the patient’s consent. Patient identifiable information may be shared with other Health Professionals, such as in the case of referral to a Consultant, and non-identifiable information may be shared with other organisations, such as total number of diabetics, etc.
Patients may request access to their own medical records by writing to Dr Harding. Requests by Insurance or Medical-legal Claim Companies for information or copies of medical records will need to be supported by a signed and dated consent form from the patient.
