Registering with the surgery

To register with the surgery:

It is helpful if:

  • you are able to provide proof of identity, ideally photo ID
  • reception could take a copy of under 18’s vaccination record from their ‘red book’

The mobile telephone number on your record should be your own personal one. For patients under 16 we can put a parents mobile number and email address on their record but when they reach 16 these contact details will be removed. The patient should contact us when they reach the age of 16 to update their contact details to a personal mobile and email.

The practice nurse will review all medical records that we receive and she may contact you to offer an appointment if she feels it would be to your advantage. If you have any questions, please ask the receptionist. We hope that you do not need us often but we will always do our best to help if you do.

We encourage all new adult patients to make an appointment to see the practice nurse for a new patient check, an approximately 20 minute appointment, so that we have a ‘baseline’ for future reference. If you are on any regular repeat medication or have on-going health problems, we suggest that you make a telephone appointment with a GP as soon as possible, so that there is no delay in providing the medication that you need.
